VIRTUAL CARE SUPPORT SERVICES

These are the steps that lead to successful outcomes when using our home monitoring app and platform:

  • Patients  are identified based on appropriate selection criteria through claims data (if provided) and/or through EMR
  • Once eligible patients are identified, they are contacted to enroll into the virtual care Program
  • Devices are deployed, which include setup and testing of the Care Trust Platform
  • Onboarding is conducted, which includes patient education, training, and support
  • Virtual Monitoring is achieved with clinical protocols and interventions
  • Clinical oversight with documentation facilitates reimbursement via CPT codes
  • Data collection and data transfer for reimbursement is fully supported
  • Program measurement and reporting metrics are provided, including patient recidivism and rationale for program withdrawal
