Partnering with Technology, these are the steps that lead to successful outcomes in collaboration with home monitoring app and platform

  • Patients  are identified based on appropriate selection criteria through claims data (if provided) and/or through EMR
  • Once eligible patients are identified, they are contacted to enroll into the virtual care Program
  • Devices are deployed, which include setup and testing of the Care Trust Platform
  • Onboarding is conducted that includes patient education, training, and suppor
  • Virtual Monitoring is activated with clinical protocols and intervention
  • Clinical oversight with documentation for CPT Codes reimbursement purposes is facilitated
  • Data collection and data transfer for reimbursement is fully supported
  • Program Measurement and Reporting metrics are provided - this includes patients that have withdrawn from the program, those that have passed away or have gone to hospice
